The Sliding Fee Discount Program adjusts the amount you pay for healthcare services based on your household income and family size, using the Federal Poverty Guidelines as our reference standard. Patients with incomes at or below 200% of the Federal Poverty Guidelines are eligible for significant discounts on all services, while those with incomes below 100% of the Federal Poverty Guidelines may receive care at nominal charges.

Our financial assistance program covers all primary care services, including preventive care, chronic disease management, laboratory tests, and medications when available through our

on-site pharmacy. The discount applies to the full cost of care, ensuring that you receive the same high-quality services regardless of your ability to pay full fees.

Simple Application Process

Applying for our Sliding Fee Discount Program is straightforward and confidential. Our financial counselors are available to help you understand your eligibility and complete the application process. To qualify, you'll need to provide documentation of household income for all family members, which may include recent pay stubs, tax returns, unemployment benefits statements, Social Security payments, or other proof of income.

Once approved, your sliding fee discount is valid for one year, after which you can reapply to continue receiving financial assistance. We review and update our fee schedule annually to reflect changes in the Federal Poverty Guidelines and ensure that our assistance remains accessible to those who need it most.

As an FQHC, we can provide telehealth services through our secure platform while maintaining the same quality and comprehensive care you receive during in-person visits.

Our providers can conduct medication reviews, discuss test results, provide health education, and coordinate referrals through telehealth appointments, making healthcare more accessible and convenient.

Our Affordable Housing resources connect patients being “priced-out,” or experiencing housing insecurity with community resources and support services. We know that unstable housing makes managing chronic conditions nearly impossible, and that housing stress directly impacts physical and mental health. Connecting patients with housing resources is preventive medicine.

Community resources that link you with services beyond healthcare - transportation assistance, job training programs, and other supports that contribute to overall wellbeing. When these needs are met, health outcomes improve dramatically.

On-Site Pharmacy Services

Our on-site pharmacy provides convenient access to medications prescribed by your primary care provider, with the added benefit of pharmacist consultation and medication counseling. The pharmacy accepts most insurance plans and offers competitive pricing for patients paying out-of-pocket.

For patients in our Sliding Fee Discount Program, pharmacy services are available at reduced costs based on your income level. We also offer mail delivery services at no additional cost, making it easier to maintain medication adherence and manage chronic conditions effectively. We can fill prescriptions from any doctor or practice.

Getting Started as a New Patient

What to Bring to Your First Appointment

New patients should come prepared to ensure a smooth and comprehensive first visit. Please bring your current photo ID and your insurance card if you have insurance coverage. If you'll be applying for our Sliding Fee Discount Program, bring proof of income for all household members.

Bring all current medications, including prescription medications, over-the-counter drugs, vitamins, and supplements, so your provider can review them for effectiveness and potential interactions. If you have medical records from previous providers, bringing them can help us understand your health history and provide better coordinated care.

For patients without insurance, we recommend bringing your most recent tax return as proof of income, or alternatively, recent pay stubs for one month, unemployment verification, Social Security statements, or other documentation of household income. All income documentation must be no older than 60 days. If you do not have these documents, please schedule an appointment. We want to remove any barriers to accessing healthcare.

New Patient Information and Registration

To expedite your first visit, consider completing new patient information packets in advance. These forms include medical history, current medications, family health history, and emergency contact information. Completing these forms ahead of time allows your provider to spend more time focusing on your current health needs and care planning. A link will be sent to you ahead of your appointment.

Our registration staff can help you understand insurance benefits, and sliding fee discount eligibility. We accept most major insurance plans, including Medicare, Medicaid, and commercial insurance, and we're happy to verify benefits and explain coverage for the services you need.

During your first visit, your primary care provider will conduct a comprehensive health assessment that includes reviewing your medical history, current medications, and any health concerns you may have. We'll discuss your health goals and develop a personalized care plan that addresses both immediate needs and long-term wellness objectives.
