Our FACT Team serves as a "hospital without walls," providing the intensive support you need while you remain connected to your community, your family, and your life. We're here 24 hours a day, seven days a week, because mental health challenges don't follow a schedule—and neither does our commitment to your recovery.

24/7 Availability
Mental health crises don't wait for business hours. Our team is available around the clock, every day of the year. Whether you need support at 3 PM or 3 AM, someone from your FACT team is just a phone call away. This continuous availability helps prevent hospitalizations and keeps you stable in your community.
Community-Based Care
We meet you where you are—literally. Our team provides services in your home, neighborhood, workplace, or any location where you feel comfortable. This approach allows us to understand your real-world challenges and provide practical support that fits your life, not a clinical setting.
Whole-Person Approach
Mental health affects every part of life, so our care addresses every part of you. We help with psychiatric treatment, medication management, daily living skills, housing, employment, family relationships, physical health, and social connections. Your FACT team coordinates all aspects of your care, so nothing falls through the cracks.

Who We Serve
Our FACT Team is designed for adults (18 and older) with severe and persistent mental illness who need more support than traditional outpatient services can provide. You might benefit from our program if you:
- Have a diagnosis of schizophrenia, schizoaffective disorder, severe bipolar disorder, or major depression with psychotic features
- Have experienced multiple psychiatric hospitalizations or lengthy hospital stays
- Have difficulty managing daily activities due to mental illness
- May have co-occurring substance use challenges alongside mental illness
- May be transitioning from a state hospital or residential facility
- Haven't responded well to traditional outpatient treatment
Referrals to our FACT Team come from various sources: hospitals, crisis centers, community mental health providers, the Department of Children and Families (DCF), the criminal justice system, family members, or self-referral. All referrals must be submitted to and approved by Central Florida Behavioral Health Network (CFBHN) at (813) 740-4811 or visit www.cfbhn.org for the referral process. During your initial assessment, the team will determine if FACT services are the right fit for your needs.
Your Treatment Team
What makes FACT truly unique is that you don't have just one provider you have an entire team dedicated to your recovery. Every member knows you, your goals, and your treatment plan. This collaborative approach means you always have access to the expertise you need, when you need it.
Psychiatrist and Psychiatric APRN
Our board-certified psychiatrist and psychiatric advanced practice registered nurse (APRN) provide comprehensive psychiatric care, including diagnosis, medication management, and treatment planning. They're available for regular appointments and urgent consultations when symptoms change or crises emerge.
Licensed Mental Health Professionals
Licensed therapists and counselors provide individual and group therapy, helping you develop coping strategies, work through trauma, manage symptoms, and build the skills you need for long-term stability. Therapy focuses on practical solutions for real-life challenges.
Registered Nurses
Our RNs manage medications, provide health education, monitor for side effects, coordinate with other healthcare providers, and ensure you're getting the medical care you need. They can administer injectable medications and help you understand your treatment. We work closely with our Primary Care Services to ensure integrated physical and mental healthcare, and our Pharmacy Services team ensures seamless medication management.
Peer Support Specialists
Our peer specialists have lived experience with mental illness and recovery. They understand what you're going through because they've been there. They provide hope, mentorship, practical support, and living proof that recovery is possible.
Case Managers
Case managers help you navigate complex systems, access community resources, secure housing, obtain benefits, coordinate with other providers, and handle the practical challenges that can derail recovery. They're experts at cutting through red tape and connecting you with what you need.
Vocational and Substance Abuse Specialists
Our vocational specialist helps you explore employment and educational opportunities, develop job skills, and find meaningful work. Our substance abuse specialist provides integrated treatment for co-occurring disorders, helping you address both mental health and substance use challenges simultaneously.

Getting Started with FACT
Initial Assessment
Once approved by CFBHN, your journey begins with a comprehensive evaluation by our team. We'll meet with you (and family members if you choose) to understand your history, current challenges, strengths, and goals. This assessment helps us determine if FACT is the right program for you and develop your initial treatment plan.
Building Your Care Plan
Together, we'll create an individualized service plan based on your unique needs and goals. This isn't a one-size-fits-all approach your plan reflects what matters most to you, whether that's staying out of the hospital, finding stable housing, maintaining employment, reconnecting with family, or managing symptoms.
Ongoing Support
Once enrolled, you'll have regular contact with your FACT team typically multiple times per week, more during challenging periods, less as you become more stable. The frequency and intensity of services adjust based on your needs. Once you're part of our FACT Team, you remain connected for life or as long as you choose. We're here to provide ongoing support through all of life's ups and downs, ensuring you always have a team you can count on.

Frequently Asked Questions
Traditional outpatient care typically means visiting a therapist's office once a week or once a month. FACT brings an entire team to you multiple times per week, providing comprehensive services that go far beyond therapy including medication management, crisis intervention, housing support, employment services, and help with daily living. We also provide 24/7 crisis availability, which traditional outpatient services don't offer.
